Can I have and submit the English-version of Medical Doctor Certificate for joining / signing up swimming pool/gym membership from the doctor /hospital from where I am now ( Singapore Clinic / Singapore Doctor) ?

You do not say where and it would likely be specific to the program/gym involved. If in Thailand suspect they may want document in Thai and/or from a medical source licensed in Thailand.
