Parental Consent form so child can get a Thai passport

Hey All,

So my wife and I live in different countries and due to this she requires a parental consent form so she can obtain our child their Thai passport.

After searching the internet to no avail recently for such a form I was lucky enough to discover one I had located and luckily saved to my computer previously so I thought I'd share with the users of this forum so they may not encounter the same difficulties I had.

The form is in PDF format and I added sticky notes to it to help future users fill out the form.

Thanks, that is usefull.

People should not forget to have the form signed at the amphur (or Thai consulate/embassy) if living abroad, before sending it to the other parent.
