Trip to Laos for double entry visa

Hi all. I know this has been discussed ad naseum on dozens of threads but one thing isn't clear to me is about what papers or documentation are needed.

I am presently in Hong Kong. I am planning a trip to Samui and would like to stay for 2 sets of 90 days (60+30 day extension). So I am planning to book a flight from HK to Vientiane or Savannakhet, whichever is cheaper, and then apply for a double entry tourist visa.

What is the complete full set of documents I would need to bring with me other than a copy of my passport to apply for the tourist visa? Is there a special application that I should use? Would they need to see proof of income? Proof of cash? Bank statements? Lease or housing contract in Thailand? What airline tickets should I buy and print? I'd like to purchase one ticket from HK to Laos on a Wednesday and then one from Laos to Samui on a Saturday. Are any other tickets needed, like a ticket out of Thailand at any time? If I arrived on a Wednesday night, went to the consulate on Thursday morning and picked up visa on Friday afternoon, and booked my flight out of Laos for Saturday morning, would that be enough time or would I be risking maybe not allowing enough time? I'd like to buy my air tickets in advance.
