DS-160, Consulate appointment, number of monthly statements required

(1) I'm from the USA, live in Chiang Mai, and am trying to complete the DS-160 form online to renew/extend my retirement visa. I properly completed the prior information (like city/state of birth - New York). But when I get to the Country of Origin (Nationality) section, the United States of America is not appear in the list. What am I doing wrong?

(2) How many months of statements does the Consulate's Office require for me to prove deposit of my social security benefits into my Thai bank account?

(3) About how long will it take to get an appointment to have the Consulate's Office sign an income affidavit, once the DS-160 is sbmitted?

(4) Last question, regarding using the 800,000 baht combo method, how many months of statements does Chiang Mai Immigration require to prove deposit of income in addition to my social security?

1.  Not sure what you are doing with a DS-16- form - that is for immigrant visa to USA but you seem to be talking about retirement extension in Thailand - which requires you fill out a Consular form of 65k per month income under oath in front of a Consular Officer.  

2.  You write what you receive on Consular form and sign in front of Consular Officer to make it a legal document.  There is no requirement for any deposits to any accounts.

3.  You do not fill out DS-160 - you fill out the form on Embassy web site - believe Chiang Mai Consulate is closed for such now so may have to do in Bangkok (check web site).

4.  There is no requirement for any statements - you should have some in case immigration asks for supporting proof but this is not normally asked.

 

As said you need nothing other than your passport to do an income affidavit at the embassy in Bangkok or the consulate in Chiang Mai.

You will also find a link on that page to make an appointment. If you have trouble making an appointment next week I suggest you contact them. I think they will help because of the closure.
