I don't understand why you think it should be translated...you're talking about a package being sent out of a foreign country into Thailand, to this address, correct?  Then you're all set.  When you take it to the post office, they will charge you whatever rate they charge for shipping to Thailand.  There's only one rate for the entire country.  The only word on the address that needs to be in English is "Thailand," and you already have that.  Trust me, that one word will get it from wherever you are to Thailand.  Once it's in Thailand, they will know what to do with it next, because the address is in Thai!  Actually, writing the address in English for a package going into Thailand could very well cause it to be mis-delivered!  Stick with the language that the postal service in Thailand reads best.

 

Seriously, I've sent countless packages to Cambodia, and Thailand, with the only English word on the package being the country it's going to.

If sending from the US, need to put zip code before Thailand, otherwise USPS service might mis-send to a US zip code 20000.

Don't worry the address will be in Thai as in the original post, this is for the custom form, the fact of a customs declaration indicates that the destination is not Thailand, U.S.A.
