Does anyone know where I can get a TB Test in Chiang Mai. My wife needs to apply for a 5-year visa and I can't see anywhere any 'approved' clinics/hospitals.

If it is for a visa you are limited to those locations which the Embassy lists as approved and it is quite possible there are none in CM.

 

For UK, Australia and Canada I believe they will accept it only from IOM in Bangkok.

If it's a 5 year visit visa for the UK then she does not need a TB test. The UK only requires a TB test if applying to enter the UK for periods of more than 6 months. A 5 year visit visa only allows the holder to remain in the UK for a maximum of 6 months per visit.

 

If she is applying to settle in the UK, then she does need a TB test, which can only be done at the IoM clinic in Bangkok; see Tuberculosis testing in Thailand.
