Heart check-up - is an echocardiogram needed?

That 'general consensus' has been disproven, and it can be up to about 30bpm in error :)

It is especially found to be in error for people who are athletes or very fit. My current state of fitness seems to be very good, with a resting HR of about 54 bpm and running 10km in 1 hour, (which is not bad at 58 years old).

When I jog for 1 hour, I never get tired.  My heart never races, I don;t get short of breath and I don't get a stitch.  I stop after 10km because that is the distance that I'm happy to train for, (as opposed to running a marathon).  Over the next few months, I expect to reduce my 10Km time to about 50 minutes as I increase my pace.

Is that 10 Km on a treadmill or road running ?

Quoting my cardiologist, "stress tests will catch 90%".  Having been (apparently) in the 10% that gets missed -and on several consecutive annual stress tests- I'm of the opinion that 5200 baht is cheap to go onto the next level.

-2005 shortness of breath, stress test, "passed"

-2007 increased shortness of breath, "passed"

-2008 july repeatedly pain left arm and chest

-2008 october CT-scan with contrast medium injected

-2008 november open heart surgery, 4 coronary, 1 aorta bypass.

All in LOS?

I love some aspects of the Thai health care system, including the affordability and not needing to see the doctor (spending $$$ and hours) to get a simple prescription.  And the level of care is outstanding once they figure out what's wrong.

But there are some less desirable differences.  Diagnostic skills are (IMO) the biggest.  Here, I suggest people get a second opinion whether the news from the doc is good, or bad.  Back home, I never felt the need for a second opinion unless the news was bad...

surgery was done in Germany because my wife said "surgery in Thailand no way. only over my dead body!"

but when i collapsed end of 2015 and had a pacemaker implanted (Bangkok Pattaya Hospital) she had no say as she was abroad travelling.

comments from the German surgeon when i was discharged. "my team and i visited heart centers in Thailand a couple of years ago. among them the Phya Thai Hospital in Bangkok. they would have done an exellent job too and most probably at much lower cost. but then you couldn't brag that (mentioning his name) has fixed you."

in Munich's Heart Center i paid for the only available "suite" which was just a normal hospital room with an attached cubby hole containing a bed, a chair and a TV for my wife the princely surcharge of 52,000.- Baht per day (room and board à la carte only).

My bypass surgery would have cost 8-10 times as much in a US hospital, paying as fee for services.  Total bill here was $60,000, but that included a lot of tests, an unsuccessful angioplasty, and probably the most expensive hospital in Thailand.  Their advertised cost of the bypass alone is a fraction of my bill, but that's the insurance company's problem.

perhaps a stent but not open heart surgery.

Suspect they may be able to open for that price - it is the closing conditions that would make me think twice before considering treatment in India - my memories of India are not that pleasant, or sanatory; but that said have had treatment from Indian nationality doctors here in Bangkok which believe first rate.

You also have to look at the small print.  What's included, what's extra, who qualifies and what conditions disqualify someone from the package price.  Then add a flight, time away from work (in my case), and the ability to travel when you need the procedure, and return after the procedure.   Plus the continuity of months of after care and exercise therapy with the same folks who did the work...  Mine came in right about 3x the hospital's advertised package price here in BKK.

I'd consider India if I didn't live and work in Thailand (and have 100% insurance coverage).  But I'd sure get more details than a price.  And I'd want to see the actual hospital and meet the actual doctors, which is a lot easier a few miles from home.

Up to you,  India for me,every time.  Three procedures done there over the years, first class every respect...and if I may say  health tourism is booming there, all over the world patients flock there, must be good

Both.  I started my training on the treadmill, but I also run on a relatively flat road.  Since it's monsoon season right now, most of my training is in the gym and I'm now increasing the treadmill incline little by little.

Good info.  It would also be great if you mentioned the city and hospital and the procedures.  I'm sure the quality varies from hospital to hospital.  "India" is a big place.

Do some research for yourself,  start in Bangalore  20 world class heart hospitals in just one city..and surgeons are generally rated there, not Thailand, could have a butcher operating here

if you insist that anywhere in India a successful bypass by open heart surgery can be offered in an acceptable hospital i withdraw from this discussion. i own property in India, spend (actually spent past tense) quite some time there, have a residence permit and i am familiar with a couple of hospitals in Mumbai and Pune.

my experience is that Indian doctors (in India and abroad) are generally excellent diagnostics and possess top notch skills. but even the best hospitals can't cope with the standards i am used to and that applies especially to stationary care after surgery.

no surprise here. i spent 2½ days in ICU, walked after discharge three times the suggested distance and had later a healthy sip of single malt. however, i had some not so funny hallucinations (remnants of the anesthesia).
