You don't register with hospital here,  you go in for a visit for a specific thing and then you establish a record of medical care

You have several options in Pattaya that are not Bangkok Hospital Pattaya.

They are Pattaya International Hospital on Soi 4, (my favorite),  Pattaya Memorial Hospital at 2nd Road and Klang (primarily caters to the Russian crowd) and the new Thai Public hospital on Soi Bukhao

You are in for a financial thrashing with all private hospitals,in and outside Pattaya.  The navy hospital gets a mention, but Banglamung state hospital is OK  and next level up Chonburi city is regarded as excellent.  Me? I take myself elsewhere

Prior to seeing a hospital doctor you register with the hospital you are going to see him at - private or government the first time using that hospital.  Many of use have a pocket full of registrations.  Perhaps you are thinking of something else (government/insurance or such) but you do indeed register with hospitals here with your passport/ID and residence/phone numbers and insurance if going to use that for direct payments.

Been here for 12 years retired, prior to that 4 years working and in both Bangkok and Pattaya, I never registered at any hospital.  Made an appointment and then saw a doctor. Hospitalized 4 times.  I hardly consider making an appointment "registering" with a hospital

True for Bangkok Hospital Pattaya, Pattaya International Hospital, Pattaya Memorial, Bumrungrad International and Bangkok Nursing Home Hospital

The first time in each facility they register you - that is the HN (Hospital Number) number that every patient has and is used in billing/medical records/medications and making appointments.  Perhaps it was done at the same counter that doctor appointment used so not obvious but I have received a credit card size paper with name and hospital number on it from the 7 hospitals I have used.  That is your registration card.

40K for a 2 night admission is not at all unusual for a private hospital in either Bangkok or Pattaya. If by Ekami Hospital you mean Sukhumvit Hospital, I assure you had you required admission the cost would also have been much higher there.

The only way you are going to get much lower costs is to go to a government hospital. Choices there are:

Queen Sirikit Naval Hospital in Sattahip

Pattaya City Hospital

Chonburi Provincial Hospital (in Chonburi town)

Red Cross Hospital in Sri Racha (Queen Savang Vadhana Memorial)

The first two have double tiered pricing so care will cost more than at other govt hospitals but still much less than private. The last 2 AFAIK do not have a dual pricing system. Pattaya City is smaller than the other 3 and has a more narrow range of specialties but fine for common problems

In any government hospital, be prepared for long waits (really long, as in often take a full day and for specialist may have to come back another day), not being able to book appointments by phone, red tape and language barriers with the admin and nursing staff, best to have a Thai speaker accompany you the first time.

For simple outpatient care, private hospital rates are usually not too costly and given the greater convenience, unless you are really strapped financially it may make more sense to use one of them for things that will not require expensive tests or inpatient admission. (Do not buy medication from the hospital pharmacy unless it is a controlled substance, there is a mark up, buy at an outside pharmacy. Also, if expensive blood tests are needed, you can have them done at lifecare labs for less).

As others have mentioned you can register at multiple hospitals, I am personally registered at around 15 hospitals. Registration for those not covered by one of Thailand's national health schemes simply means receiving a patient number that links to your medical records, nothing more.
