Normally hospitals have eye clinics with qualified opthamologists and testing facilities and can provide you a prescription (and check for medical conditions) at very reasonable cost.  You can then get made from chain or much cheaper in most cases by using mail order from a place like Zenni.  If using mail order have doctor include   PD on your prescription (distance between eyes) as they sometimes do not bother) - you can do yourself however if need be.

They are really inexpensive. Does not work for complicated prescriptions. But, for a single prescription, for distance or reading, they are outstanding. I have purchased lenses with frames from them for a little as $25. Forget the retailers. The markups are astonishing.
