Where to download driving licence application form

Hi all,

 

I'm due to renew my driving licence soon (5 year to 5 year) and I've read in various places that it saves time to download the application first as it's in Thai. Does anyone know of, or can send a link to, where this can be done? I've not been able to find anything through various searches.
