Just the first page  with photo and info and whatever pages pertain to Thailand.

Although the hunger by IO for useless - each year returning - paper copies is huge, they are not (yet) so crazy as to also request copies of your EMPTY passport pages (that would add easily 30-40 pages of paper copies to an application so the prospect might be tempting for them).

So short answer NO > As a minimum = copies of passport ID-page, page with original Visa, and page with last permission to stay stamp OR as a maximum (required by paper-hungry Immigration officers) = copies of passport ID-page as well as all USED pages with stamps pertaining to Thailand entries/applications.

Normally the photo page, the last visa,the last extension, entry stamp and the departure card (TM6). But then some offices asks for more for some strange reason. 

To keep me from having to run to the photocopy shop I provide a copy of all stamped pages. As usual, the requirement changes from day to day as well as who the IO is.
