I have to go back to the U.S. in April for business. I have a appointment at the VA hospital for a vaccine shot. Different travel agents are quoting a Philippine Airline flight  $650 twice a day to Los Angeles. Is this the best flight? What are my options coming and going? Will I still need to quarantine and get health insurance? Thanx alot for your help.

Yes and yes.

BTW if things go on like now, by April you may need 3 weeks quarantine...depends on the development of UK and SA variants in the US. HK and Taiwan already have reacted quite drastically. 

Philippine airlines is presently the cheapest option. Note however, that at the moment, they have a single flight each week from Bangkok to Manilla (on Wednesday) and 4 times/week Manilla-LAX. This can change at any moment, but published schedules for April don't mean anything at the moment. Check https://www.philippineairlines.com/en/ph/home/covid-19/reentryflights to see impact of COVID and  https://www.philippineairlines.com/en/~/media/files/flighttimetable/international winter as of feb 10 2021.pdf?la=en for present schedule (extremely limited). It is also a good idea to check whether you can transfer at MNL without problems.

Whether PAL is the best choice is difficult to say; privately I would prefer EVA or a Japanese airline (ANA/JAL) but these cost more. Whether these will actually go is anyone's guess. 

 

Transit flights through Taipei were forbidden Dec 31, I don't think they started again. 

That rules out EVA.

Ask these travel agents if you will be allowed to transit in the Philippines.  As things stand currently, most people can not.

during the first wave some consulates were publishing updated available flights.

check with the usa consulate if they have info or point you.
