On the part on the bottom where they ask about your visa. They do want the info from your original visa and not the last extension?

I assume you are asking about the TM8 re-entry permit application form.

It seems you may be using a old form. The info for the visa is requested at the top of page 2 now.

It is asking about the visa you used to enter the country.

I use the visa number( not extension number) I had when entering the country on the new visa many many moons ago.

An extension is a permit, not a Visa.

That was the part that had me confused. As it turned out I did have the wrong form (thanks Joe). I went in today and "they" put the info from the last extension.
