Just read that 3 million Covid cover will be needed in order to renew your Visa or extension to stay in the future, is this for new NIV applicants or for all including existing NIV holders? My Pacific Cross insurance covers Covid but no where near the 3 million, 

It is only for those that have valid Non-OA visa or the extension of a entry from one based upon retirement.
