I have an extension of stay and want to travel outside of Thailand and return I understand that I need covid insurance is needed until my extension of stay expires. Has anyone done this yet and I would guess that upon arrival at immigration at the airport you would need to show the covid 19 insurance but do you need to show it again going forward at your 90 day appointments?

It is required for the CoE.

And it would be verified at check-in/transit and on arrival in Thailand  (Imm, health/covid).

After that, I wouldn't think it would be required for a 90-day report, or an ext of stay.
