91 years old. Almost bedridden. How do I get a Booster shot?

I am 91 almost completely disabled. I have six lung diseases and I would need a booster shot now

I was able to get to the hospital to have my first two shots. But my disability has increased since then and I am almost bedridden so how do I get my booster shot
