A letter and proof you cannot do it yourself.

No it isn't. He wife can go for him. All she needs to do is take a photo of him in bed/wheelchair. I see no requirement in the TM7 form for permission having to be provided though a simple letter in Thai signed by the Op wouldn't go amiss.

 

Dear Immigration,

I am very sick/disabled and I can't come to Immigration for my retirement extension. My wife has got my completed TM7 and other documents for your kind consideration. 

 

Don't forget the 1900 baht, letter from bank, passport type photo etc.
