I try go tell them initially that I don't need the medication before they print up the bill. I find it easier to just say  I already have  the meds.

Actually try to forestall it by telling the doctor so he knows not to order any.  But sometimes things like paracetamol etc get gratuitously tacked on.

In more than 3 decades here, never been unable to avoid meds from hospital pharmacy. But if one waits till  bill is already printed it does mean extra time/delay.

I do it all the time, at Bumrungrad.

Just stand your ground and keep calm and repeat that

'it's now the law in Thailand patients/customers can buy their medications outside of the hospital'

The law applies to all (repeat all) hospitals.

Same here... When seeing the Doctor its easy enough to just tell them you want to buy the meds outside of the hospital - I’ve never had pushback on this.

...he indicated a desire for a second opinion which suggests a need for a doctor with socialized expertise in shoulder injuries.

Sorry, Sheryl, he made no reference to needing "socialised expertise" [sic]...

"Can anyone recommend a good surgeon /doctor in Bangkok for treatment and follow-up of the fracture?"...

he just asked for recommendations for good doctors who could examine, as a second opinion, an injury as basic as a broken shoulder and he was given that information with plenty of alternatives, including the hospital that you named.

If you are a tourist and if you paid for your flights with a credit card then check your credit card providers T&C's  because accidental injury insurance is sometimes automatically inclusive when flights have been purchased.
