Seem to be hearing about more driving arrests these days which may suggest that police are finally starting to do their job. Probably going too fast or saw the red light too late to stop. Hope the authorities investigate properly and prosecute accordingly. RIP the road worker.

Even if there is an extremely rare instance where the brakes are at fault, whose fault is it?

The vehicle owner is responsible for replacing brake shoes in a timely manner, and for other basic maintenance.

Do their job. They are arriving after the fact. Where are they to check for driving misdemeanors tand prevent accidents?  How often are public service and good vehicles checked for road worthiness?
