I saw reports a few years ago that re-entry permits were available at Phuket airport but, unlike at Suvarnabhumi and Don Muang, could take a long time.

yes, after u exit immigration is the office or just ask the official at the desk> if at off hours, they will run down someone to do it, give urself an extra 15 minutes
can also get one at Patong or blue tree immigration not just town

When I was departing this week I noticed a sign to the far left (next to the air crew/diplomatic immigration lines) that said "Overstay and Re-Entry Permits". I would assume you'd need to tell the person directing people at the queue that you need to go there rather than the regular immigration line.
