Generally speaking, no.

A non O-A and any extensions from a non O-A based on retirement will require insurance.

Extensions from a non O-A based on marriage do not require insurance.

Non O visa and extensions both based on marriage or retirement do not require insurance

Not necessarily.

It's based on his ORIGINAL visa before annual extensions.

It will be either an O or an O-A visa.

If O, no insurance needed.

If O-A, insurance needed.

O is definitely better to the point where many people with O-As have started over with an O. 

In my case, on an OA (Retirement) - yes, has always been enforced since it became policy.

Surprised you have not exited (border bounce) without reentry permit to kill off the non O-A

Enter  visa exempt to "start over" 

No insurance required.

Many folk have opted to do that.

Don't know what oap is.

You need to be clear.

 

Edit:  oap could mean old age pension.

That has nothing to do with Visa

But would you not first have to have done 1 extension based on the reason for issue of the visa, (over 50), before you could change to an extension based on marriage ?

Agent OA extension, insurance adds 3000bht to the envelope, accident insurance it is tho, not health ins.
