Yes there is a genetic issue however, if you read and study the firs things that they recommend is dietary care, exercise medications

The intervention of dieticians, psychologists and teachers of adapted physical activity is recommended for every patients suffering from genetic or syndromic obesity or at-risk of severe obesity later in life, in cases with early diagnosis 

 

These are things that are well beyond the average thai family to afford especially if it is just a 66 year old grandfather that knows amost nothing except his GS is obese.
