I lost my Thai 5 year Driving license yesterday. :o

I have already been to the police station to get the report document. I want to know what I will need to take to the Licensing centre to get it replaced. My address has changed since the license was issued, so I am guessing I will need to go to Immigration to get a new Address document ?

Anything else ?
