Can someone explain the last requirement (9) for an extension of an 'O' class visa it reads:-

Item 9 "Addendum of Inquiry for both husband and wife"

If this is for support Thai wife (you did not mention reason for visa) it will be an interview of both parties where extra bio data and history are recorded. This is informal at desk type asking of questions and filling out form in most cases.
