I do have a "surefire" way (aka, it did wonders for me), but it takes an effort of perhaps epic proportions. PM if you want to follow what I did over the course of 8 months.

The only thing I can advise about, diet wise, is the high blood pressure, what with me being long time sufferer. The most important thing is to cut down on salt. In Thailand this basically means cook your own food, or at least advise restaurant to take it easy with salt. Rice is not bad, but most any kind of curry or soup here has a lot of salt. Also try to stay away from sausage, pickles and sauerkraut on the farang side. Also, it is advisable to limit your alcohol intake. I know it is not a dietary subject, but aerobic exercise of 30 -40 minutes daily will help with all of the problems you mentioned.

What is you cholestrol level and are you over weight. A magnesium supplament can help to lower blood pressure most people have a low magnesium level.

Avoiding processed carbs is critical.

Getting the diabetes well controlled will in itself help lower the lipids.

Beyond that, high fiber and limit intake of saturated fats, increase intake of "good" fats/oils eg eat lots of fish, use olive oil.

Should note that people vary greatly to their response to dietary intake of cholesterol. Some are what are termed "responders" and will improve when intake of LDL-C is restricted (and the opposite) while others are termed "non responders" and show no particular change regardless of intake. Won't know which you are until you try a good diet for a few months.
