Lost My 90-Day Report Form

I've lost my last stamped bottom section of the TM-47 90 day report form. Can anyone please advise me on what to do next? I have to report next later this week I think.

Fill out a new TM47 form and go in and report. Just tell them that you lost the old form. They should be able to look it up on the computer, if there really is a 90 day report file. I don't think that they will require a police report, but who knows?

F.y.g. i have been in exact the very same situation as you for about 2 years ago. The immigration-guy saw it, I didn´t, and said that as I was missing this slip of paper, and I have to go up to the place where I got it from and fix a new one up there...... 700 km two ways....

Thereafter he offered me a chance not to go all the freaking way up to Chiang Kahn if I paid him 2.000 baht. My wife came into the office, and he suddenly went down to 1.000 baht... I was panicing almost, and accepted the freaks offer and paid him this 1.000 baht, and he just fill in the form, stamped it, and clipped in into my passport and done.....

This was Khon Kaen, I remember that I publiced this incident in this forum, if this has anything do do with it, I do not know, but next time, 3 months later, this guy was replaced and the whole Khon Kaen office hasen´t been the same since.... They are VERY good and correct now, no bullshit to my knowledge with bribes at all, at least not in my case.

Immigration keeps computer records of 90 day reports. Plus they file them away.

You should not have to pay any fine unless they cannot find your report in computer or storage.

The immigration office misplaced my receipt for report during process of doing my extension. My last report was missing from computer. After questioning them about report being filed away they brought stacks of reports for week I made report. Each stack had computer print out of reports made on the day stacks were for. Wife found my name on computer list and original form. No fine paid.

Phuket Immigration is using the papers for the copy machine :-)

I am not sure how long they keep them but I would think at least 90 some days.
