I want to take my 3-yr-old Thai/American son to the USA on one-way tickets. He has both American and Thai passports. My Thai wife seems to be OK with this, but she is iffy and unpredictable. 

When I pass through passport control/immigration at Suvarnabhumi Airport I want to have all the absolutely required docs in order to eliminate any chance of a snag.

I'd appreciate any insight on this. Must travel by end of April.

There is a Document provided by your local Amuphur office. 

Your Wife will have to sign it....  I forget the name, but is An Affirmation of Freedom to travel, notorised by  an official at the Amphur office. 

 

The document basically states that your Wife has given you permission to travel internationally with the child.

Do you have the name of this form in Thai, or a code for it? I understood a letter would be sufficient, but if there's an official form that would be great.

 

Also, would I be able to go through the Thai channel at Survannabhumi with my (Thai) daughter?
