Notary Public in Rayong?

Does anyone know of a Notary Public in the area of Rayong where I would be able to have a document notarized? I have a SWORN STATEMENT form that I must send home to verify that I am still among the living. This form must be filled out and signed by me and have my right thumb print on it and then be notarized.

As no replies for you so far, I can recommend a NP based in Pattaya. I have used him for a similar 'life' form requested by the UK state pension office.

This is at Euro Thai Law office, opposite side entrance to Big C at corner of Sukhumvit Road and South Pattaya Road. If needed, I have a telephone contact for the office manager, Ms Rina, who speaks perfect English. Charge was Bt500 last time I used this service.
