Necessary documents for transferring green book?

I see a lot of different information, and even calling the DLT yields different results based on who answers the phone.

 

What is the most up-to-date documentation needed to transfer a green book from a farang to a Thai.

 

From my understanding I have to copy and sign:

Passport

Visa

Work Permit

TM 30

2 Paper forms obtained from the local DLT office. 

If you are not going yourself to DLT office also need the signed power of attorney form, need a 1 baht stamp attached too...!

Where do I get this?

At DLT offices there is usually a photocopy guy next door that does all the photocopying, he had this stamp and gave it to me, my seller filled and signed that form but didn't stick a stamp on it...

Is a residency certificate required or will a TM 30 suffice?

That depends on which DLT office you go to, in Phuket they didn't want any proof of residence from seller last year I used them, only from buyer, but I read some other offices want it. Check this forum a few months back, the guys from Bangkok and Pattaya mentioned this....
