In Thailand the common transliteration for Monk into English would be Bhikkhu, Bhikshu only being used with the Mahayana sect.
But i guess that you have ordained in the Theravada tradition?
Is this a forest monastery or a village monastery?
You do mention Udon Thani, is this the province you have ordained in?
There are quite a number of answers regarding the Covid extension already,
so here is what you need for the 1 year monks visa (called Non-Immigrant type R):
 

Dhammayuttika Nikaya.

I know quite well about it. I just happen to like how the words sound in Sanskrit. Bhikṣu is far more beautiful the Bhikkhu.

It's a village monastery despite it has Pa in the name, lol.

Today I went with the Ajahn to another monastery to start the process of obtaining the 1 year visa. We went to see, apparently, the "DjauKana Tambon". We'll be back there tomorrow to get the signature.

I took the picture today!!!

He already asked for the bai sutthi. I heard that it'll be ready within a week.

 

I have two doubts. Can you clarify to me?

Must the certificate of residency state that I live here? The Ajahn gave me a document with the address here. Signed and stamped but no mention of me. Is it enough?

 

Where is this "Udon office of SamnagNganPraPuttasasanaHaengChaat."?

It is in the town hall north of ทุ่งศรีเมือง (Tung Sri Mueang) public park on the 5th floor .
ศาลา กลาง จังหวัด อุดรธานี ชั้น 5 ถ ต หมากแข้ง อ เมือง, อธิบดี ตำบลหมากแข้ง อำเภอเมืองอุดรธานี อุดรธานี 41000
Everything will be explained to your Ajahn in detail once you go there.
Usually open Mo-Fr 09:00 until 16:00.

No, they are two completely different things.

-Bai Sutthi=Monks ID, and yes, it has your "Prawat (meaning history of your monks life) documented inside.
Ordination, Preceptor, where, when, your personal details and so on.

-the form "Prawat" for the SamnagNganPraPuttasasanaHaengChaat is much more elaborated and is only for asking for a monks visa.

Once you have been to SamnagNganPraPuttasasanaHaengChaat in Udon City together with your abott everything will become clear.

I'm only 21 years old. There isn't much to put in a curriculum. I'll put I finished high school and I don't know what else. ????

But anyway, I'll tell Ajahn about this.
