Hi
I recently received at 60 day extension at Chiang mai immigration. As well as the usual photocopies and the TM7 form, they gave me an additional form to fill in, and sent me to the back of the queue again for my sins.

 

Form was Thai with English translation in same style as TM7 and I forgot to take a note of what it was.
 

In order to avoid getting sent to the back of the queue the next time I apply for an extension, I wondered if anyone knows what form this was.

Was it a single page form with Thai at the top English on the bottom half of the page that is a STM.2.

Was it titled
