Just the completed 'Transfer stamps to new passport' form, both passports, copies of all pages of the old passport, copy of data page of new passport and the entry stamp.

Thanks.  Thailand following UK in wanting ALL pages of old passport.  Why they want photocopies of blank pages is beyond me.

Not all immigration offices do.

Certainly CW does not.

Here is list on "transfer form"

Obviously tm6 is no longer required.

Note: in addition some offices asking for updated bank book and photocopies of pages back to date of extension.
