We are using Authorize.NET, it's pretty good and support is OK. Not sure about the Thai bank thing as all our clients are in the US.

We use 2checkout.com, and can't complain about their service. The fees are 5.5% plus 30 cents per transaction. No problems with a thai bank account (even though we dont use one).

I think PayPal is the same no? I read in the Money/Investment/Job forums on Thai visa that they now offer service in Thailand - maybe worth checking out?

PayPal doesn't offer withdrawal of funds to Thaibanks as of yet...
