I have looked into completing the new arrival-form, but it does not currently allow me to give my country-of-residence as being in Thailand, how should I complete the entry-form in this case ?  Of course I don't want to give an incorrect address/country-of-residence, to you.

 

I will return to Thailand, from my next trip overseas, on 13th May  ...  and hope that all will become clear, by then.

I am in the same situation. I travel by road to hospital in Khon Kaen on Sunday 4th May via the O'Smach - Sisaket land crossing. I shall plead ignorance and if I am refused then I will discuss the need for urgent medical treatment. I only have an old 3G mobile. This crossing is usually quiet with mainly local Cambodian and Thai travelers. Maybe they still have a collection of old TM6 forms available.

 

Recently, I travelled to the same hospital but travelling with Air Asia from Siem Reap to/from Khon Kaen. Thai Immigration in Bangkok (DMK) were brilliant. Being in a wheelchair, I was kept clear of all the channels with passengers queueing and was taken to the DIPLOMAT- VIP channel and processed in a couple of minutes. I was the the sole traveler who was then ushered to the right of the Diplomat channel to a hidden lift within the Immigration Admin and rest area and taken to the ground floor. Thankyou Air Asia and Thai Immigration - first class service!

There are other ways aside the phone to do the TDAC.

Yes, thank you. I will try using the website on my computer and generate a paper copy for presentation to Thai Immigration.
