I notice that most traveling outside of major cities aren't wearing helmets besides the actual motorcycles, and not scooter drivers. I don't see many foreigners driving motorbikes where I live, but the ones i do see, the majority aren't wearing helmets and are on scooters. The younger crowd it's mostly no helmets, meaning locals of course as young foreigners are the tiny fraction here besides the tourist areas. In my village, I rarely see anyone with a helmet on.
