If you are talking about a Multi Entry Non Imm O Visa obtained from a Thai Consulate you can do a border run the day before it expires and get another 90 days. If it has expired you need to go to a Consulate and apply for another one.

OR

You can apply for a 12 month extension from Immigration in Thailand if you meet the financial requirements.

(6) In case of marriage with a Thai lady, the husband who is an alien must have an average annual income of not less than 40,000 baht per month or a money deposit in a local Thai bank of not less than 400,000 baht for the past 2 months for expenses within a year.
