The cost is 1,900 baht for form TM.7 at immigration. You also may need medical certificate, bank letter and pension verification from Embassy so they would be extra. A few photo copies will also be required. The same requirement of 800k in bank or pension of over 65k per month with a bank account having some money in it.

No you can not do your 90 day report online. You can do it by registered mail however.
