I got a very good rate at a small booth next to the Tai Pan disco on Rat-U-Thit road when I came in June this year.

Unless you plan on exchanging a serious amount of money for your holiday, just get it changed in Patong and skip the trip to Phuket Town. The tuk tuk price will wipe out any additional currency gain.

any bank for small amounts ( under $500) if more, Phuket town

or the exchange booth 2nd floor Central mall ( where all the banks are, booth s easy to find)  before phuket town
