 I worked in Thailand 2007-2013, I guess thats irrelevant now... ?

1. Do you submit 1 x form and 1 x set of backup documents (as they list) or 2 sets?

2. How many photos?

3. anything else required  (for example copies of every page with information in the passport)?

I've downloaded the forms from the https://bangkokimmigration.com/2020/09/24 link Ubonjoe gave on page 1 of this thead

 

Its' a long way to travel to CW to be told to bring something else, so anything i've missed/need?.

As far as I know only one form and the required documents is required.

Have you downloaded this file and checked the list of requirements for the category you are applying for.
