Basically you have to be working with a work permit, paying taxes and be on extensions of stay for 3 consecutive years to apply.

 I worked in Thailand 2007-2013, I guess thats irrelevant now... ?

Working ! That's me out then, no thanks got enough to do as it is. 

1. Do you submit 1 x form and 1 x set of backup documents (as they list) or 2 sets?

2. How many photos?

3. anything else required  (for example copies of every page with information in the passport)?

Its' a long way to travel to CW to be told to bring something else, so anything i've missed/need?.

As far as I know only one form and the required documents is required.
