You may get a more accurate answer. 
 

Example: Wife got hers from DLT area 3 (Bangkok, Sukhumvit rd).

 

I think she had to book an appointment online.

I have obtained them at Bangchak and at Morchit. No appointment, just walked in.

 

2 Photos and original 5 year Thai driving license, pay 505 baht fee.

 

I guess for a Thai national their ID card and maybe copy of house book would suffice.

Generally yes. I have used mine for renewing driving licence, buying a car etc. However you might meet Mr Difficult who wants to apply his own version of the rules ...
