Hopefully references based on 1st hand experiences.

You can get a living will (called Advanced Directive) via the Chiang Mai Expats Club website (Resources tab, then CEC documents tab).  While technically allowed under Thai law I'm not too sure how effective they are (meaning whether local doctors will or won't follow instructions from your health care representative).

Thai recognized Notary Office: PTA Pattaya... Caroline Wang

Highly recommended...

I go to her with all my Paperworks incl official translation documents..

Very helpful Reggie considering the OP posted in the Chiang Mai forum.

I use Sumalee Jennapa, a very experienced English speaking Advocate/Solicitor in Chiang Mai. Here website is www.29tanin.com. She has done numerous things for me including a will and a living will.

Some Thai hospitals have a living will form, I think it was Bangkok International hospital on the super Hi-way that I got one from.

My friend gave a verbal living will to a Doctor & Nurse at McKeen hospital, they honoured his wishes perfectly despite some objections from his wife.

I highly recommend McKeen hospital for anyone who's time is drawing near.

Yes, I’ve used Khun Sumalee Jennapa also - maybe 3 times over the years. She is thorough, professional and kind. Her English communication skills are excellent.
