Retirement extension, five year license

House book, Lease agreement, ( a letter from landlord not sure about that), 

Passport, copy of TM30, 2 2x2 photos, copies of PP first page, visa page, departure card TM6 Sign all copies.

Not sure about Rayong but this is generally needed at all IO.

You will also need a Med. cert when applying for a new license go after your birthday to get 5yrs 11months.
