Live in Chiang Mai.  Have 5 year Thai license.  Is a certificate of residence form required or is my Thai license address adequate?  

Google shows some states ask for it.....

I never have been stopped or asked, but my wife & I get one....

We've both rented cars with our Thai DL's with no problems....

I have a specific purpose in mind for it during our upcoming trip....
