    OP should try another branch as this is Thailand and rules are made up or applied on a daily changing basis depending on the mood of "god" behind the counter.

In reality immigration often does not require all listed docs.

Similar but not exactly the same  Jomtien form does not have the bit about address on the bottom...There are other forms that Jomtien insists on issuing 'special' versions  and they will send you away to fill out the form and queue up again to get your ticket number.

Many years using Bkk Bank and only ever ONE mishap, where WISE sent it to Kasikorn, so I had to get Credit Advise. No problems at Immigration.

Is her name in Thai text or English text on her account.
Unless the name matches identical in the same text as on her account, it will be rejected.

I went to Kbank today. A very pleasant experience. Passport and both DL was all they required, no COR. ATM card 300 baht.

That was, exactly my experience 2 years ago. 

I was (still am) on annual extensions. 

Just needed my PP, no COR. 

I provided 12 month lease but hardly interested in it. 

Great that you had a "normal" experience today.

I had a similar experience a while ago with Kbank on a non-immigrant visa. Initially they refused to open an account with just a passport and rental contract and asked for a work permit/house book. But once I shown them my Thai driving license, they approved. Their KYC procedures were satisfied simply by having a second form of ID. Same with TrueMoney Wallet, they only approved me once provided with a Thai driving license.
