There's a mosquito bar in Lamai also. If you were driving in from Chaweng you would first veer off to the beach road by taking the left (by the Mc'Donalds sign). Follow the road straight across the crossroad and over the bridge. Keep going straight for about 300 metres. On the left on the first story just as the road bends around slightly to the right there is the Mosquito Bar.

Yes mate, in Fisherman's Village on the inside about 150m down.

Lost their card but try via baantropicana.com, lovely bloke by the name of Jon owns both I think.
