I can only say the first time it did not check the address, it stopped at phone number missing..., on 2nd run it passed the missing fields checks, then found issue with address ... I suspect the address didn't exactly match the one used for your TM30, they must have a computer auto check now before a real person sees it, so if have access to TM30 exact wording, try that...
