Go to any hospital and tell them you need the health certificate for drivers license, they all know of this. They charge less than 100 baht and don't even look at you.  I think you need a form from immigration, ask them when you are there. Then since you have a valid drivers license from home they will only need you to do the reflex and vision tests at the drivers license office. This is supposed to change soon where you will need to take all the classes, written test and driving test so I recommend you get on this as soon as possible.  Once you have this drivers license you don't need to carry your passport as your passport number is on your license. I keep my passport in my suitcase at all times anyway but have never needed it for domestic flights or hotels. You will get a one or two year temporary license and then when you renew you will get a five or six year.  Good luck.

^I renewed my driving license in Udon recently, they had no interest in a medical certificate and gave it straight back to me, you need proof of residence, possibly the easiest way is to get from immigration at a cost of 500 baht, then you have to be prepared to sit around for a couple of hours watching a video and do a colour blindness test. First license will be for a year, thereafter you can get five years.

I received my first Thai Driver's license in October 2016. It's effective for two years. Expires in October 2018. When I received this initial two year license I had a Type O visa ME (before I received my retirement extension). Also be aware that they wanted some official verification that my operation "class" signified by a letter was for automobiles.

As with most things in Thailand they constantly move the goal posts! Unfortunately the only way you know exactly what they want is by turning up!

They have obviously increased time period for first license, I have been on five year ones for some time now, it used to be a year - if I remember correctly!

Since March (23?) 2015 temporary licenses are valid for two years from issue date,

 

Just got my standard motorcycle license valid until March 2023.

(temporary was from July 2015)

The "almost 6 years until birthday" still works.

I had no medical certificate and was sent to get one. Then when I presented it they just handed it straight back to me also. Would have been much easier to have it in the first place as not to go to the back of the line again. I got five years, 11 months and 24 days.
