To convert my British Licence to a Thai one I need an "Affirmation of residency".  The British Embassy says to prove my address I need:

 

"a driver’s licence, utility bill, house registration book, rental contract showing your full name and address"

 

I do not have any of those.  Does a bank book qualify?  It does not fit into any of those.  Can my wife just write me a rental contract?   Would it need to be notified or something or can it just be scribbled on a scrap of paper?

That's a question that only the Embassy can answer, why not ask them?

 

The alternative is to go to Immigration and get a certificate of residence for a driving licence from them.

As JW said go to visa office. Your wife should have reported you as living at her place, so the Visa office will have all the details.

If she hasn't done so she's in for a big fine. If she has you should have a slip of paper pinned into your passport to confirm same.

The OP needs to clarify his current Thai visa status as well as where he lives since different, regional Thai DL offices, like Thai Immigration Offices, will have wildly different ideas about what official bits of paper one needs to get other official bits of paper and ultimately a Thai DL.

 

I also reckon if the OP is already married, getting a 'registered' home address is needed before getting a Thai DL... which will need proof of the Thai home address.

Scaremongering much?

 

Not all Visa Immigration Offices have the same draconian approach to keeping tabs on their foreign guests.

 

However, knowing the OP's current visa stratus and location will get him some less scary and usable advice.

I'm not scare mongering. I was just stating what should have been done. So come on then what's unusable with my advice. If he has the form and it's stamped and signed it should be fine.

 

N/B all Visa offices now use the same system, if that's draconian so be it.

Yes, all IO's are on the same system but some are more ardent than others in fining the forgetful.

I put that bit in to remind others to keep up to date with the rules. You have 24 hours after arrival to register where your living. That can be stretched a bit if you arrive say on a Friday night then the next visa open day is ok. 

A few days late and you'll get away with it after that most places will fine you. They have got very strict with the new rules now.

And you are dragging the OP all over hell's half-acre with talk about Immigration reporting within 24-hours when all he wants is information on an affirmation letter. FWIW, the need to do the 24-hour check-in primarily depends on a couple of things like what is his long-term visa/extension status and what IO he reports to. So far there's maybe only a couple that apply a blanket, 'everyone must report within 24-hours' rule.

 

Plenty to do, nailing one piece of useless disinformation at a time.
