As a male the odds are in favor of us not getting HIV but there is a much higher chance of the ladies getting it from us if we are positive. I was speaking to someone yesterday who claims he never uses condoms. Personally I judge each situation on merit. 1

These are drugs which, if taken daily in pill form, prevent HIV infection. This means that men can have unprotected vaginal sex with bar girls without any risk of contracting an HIV infection. Once someone has been taking the drug consistently for seven days, it then offers the full protection. This has changed the game a lot. PrEP drugs wont prevent any of the other STIs, but the majority of the other ones are treatable and non life threatening. Perhaps the guy who told you he isn't using condoms is taking PrEP. I would be careful with regular use of Doxy for PEP. Using it once or twice a week can disrupt gut health by killing off too much of your own good stomach bacteria. I think better to just deal with an STI infection if you get one after the fact. Usually a single, one-gram dose of Azithromycin (Zithromax) will wipe out most bacteria based STIs within 24-48 hours.

Between age 15 -49 yo, over 50% of the world's population have oral herpes. In the same age bracket, 13% have genital herpes. Herpes is treatable for symptoms. It is incurable. Many people are also asymptomatic and have never experience any symptoms. Some get an Initial outbreak or two and then never get it again, but are already carriers. That means you could already have it yourself and not know it if you’ve contracted the virus, but never had an outbreak. The risk of getting it from kissing is also a lot higher than from sex because it is the more common form. Even people who exhibit no symptoms at all can still transmit it orally during viral shedding periods. And it can still often be gotten by men during sex when wearing a condom because, if the virus comes in contact with the skin at the base of the uncovered penis, it can still be transmitted. Trying to avoid it is pretty much a zero sum game unless you don’t have any sexual contact at all starting from a very young age. 1

And STD's in general, probably a side effects of the AIDS scare..

I have only had condomless sex with five women in my life, after knowing them for an extended period. I don't put much credence in your statement the base of the penis is vulnerable to transmission. Do you have any links to back the claim?

But could do the Azithromycin and Doxy regiment too. In many cases that will get the job done without needing Ceftriaxone. And just get a syphilis test 2-3 times a year to be even more sure. But you feel comfortable I assume with PrEP as adequate protection against HIV infection from having unprotected insertive penis to vaginal sex?

Yep, their studies didn't include that, but insertive vaginal sex has got to be lower risk than anal sex where blood is possible
