Popular Post edwinchester Posted Monday at 09:50 AM Popular Post Posted Monday at 09:50 AM Anyone else had to fill one of these in for an extension of stay, marriage? Unsure of whose details go where as it's another one of those dreadfully put together Immigration Forms. 3

From random internet search.... "fyi - Just finished marriage visa extension at Div 1 Immigration in Bangkok - Chaeng Wattana. They requested a new form STM 10 affidavit signed by Thai wife in addition to everything else." So have wife sign it and go with the flow. 2
Equatorial Posted Monday at 10:48 AM Posted Monday at 10:48 AM The STM.10 form is available online in this collection of forms: https://chiangmai.immigration.go.th/wp-content/uploads/2025/04/ข้อกำหนดคณะกรรมการติดตาม-1-2.68.pdf
johng Posted Monday at 11:13 AM Posted Monday at 11:13 AM There were more forms to fill in this year at Jomtien I'm pretty sure that this was one of them even the immigration officer commented that there are more forms this was after the rain forest in duplicate was submitted and the fee paid....at this point quite frankly I was in autopilot and just wanted to get the hell out of there. 1
Liquorice Posted Monday at 01:01 PM Posted Monday at 01:01 PM My wife was first presented with and asked to complete that affidavit in 2022, and again in 2023. Last year it was presented in a new format from a template on their PC, with all the details included, just required signing. 1
Tod Daniels Posted Tuesday at 03:16 AM Posted Tuesday at 03:16 AM Chaengwattana has been using that form for quite a few years. They hand it out to you at the document check stage (Or at the officers desk) you just write your name on it and sign it It's in the packet of forms that have the overstay acknowledgement, the terms and conditions to your extension, and that "statement" (the STM10 This is the one from Chaengwattana 1
connda Posted Tuesday at 03:26 AM Posted Tuesday at 03:26 AM Probably. In addition to the stack of paper I bring to those "extension" appointment, but IO generally add a few other documents to the even larger stack of documents that are plopped in front of me to sign. Dang, I need an auto-pen like Joe Biden.
Caiman Posted Tuesday at 03:56 AM Posted Tuesday at 03:56 AM In answer to the OP, my assumption: Top section, Thai wife's details? Item 1; Husband's details? Item 2; Husband's details? Item 3; Husband's name? Item 4; Wife's occupation and signature. Please correct me if I'm wrong, thanks.
Popular Post Caldera Posted Tuesday at 05:25 AM Popular Post Posted Tuesday at 05:25 AM They really love their useless extra forms and papers. I'm just waiting for the first report of someone getting asked to print their two-page TDAC PDF document when applying for an extension. Nonsensical as it is, you cannot help but suspect it's coming. 4
Popular Post AhFarangJa Posted Tuesday at 05:37 AM Popular Post Posted Tuesday at 05:37 AM One more reason I changed from marriage to simple retirement. Only downside is the money in the bank. 1 1 1 1
trainman34014 Posted Tuesday at 09:11 AM Posted Tuesday at 09:11 AM Wife has been filling in and signing one of those for several years. It's usually stuck in front of her while other papers are being checked by the Officer; nothing new in Chiang Mai. 1
GreasyFingers Posted Tuesday at 10:45 AM Posted Tuesday at 10:45 AM Interesting, never seen one up here.
Upnotover Posted Tuesday at 10:50 AM Posted Tuesday at 10:50 AM I had something very similar on my recent retirement extension. I could not work out what it was about as it seemed totally irrelevant to my extension. IO just sign sign it, which I did. It does make you wonder what/who is creating these things and for what purpose. 1

Dang, I need an auto-pen like Joe Biden. When I had arthritis 2 years ago I could hardly hold a pen to sign all the paperwork. The immigration officer recommended getting a stamp made in my name for the printed part which cut the writing in half and speeded things up a lot. Now wife stamps my name and passes each paper for me to add my squiggle. Stamp cost 200 to have made locally. 1

Just waiting for mine to return from Bangkok
